Midwest Sinus Allergy Managed Care Participation
Dr. Denninghoff is a participating provider in a number of managed care plans. We encourage you to check with your managed care plan to ensure that your physician participates with your plan and that services will be covered. It is your responsibility to know the requirements of your insurance plan.
Patient Financial Policy
We are dedicated to providing the best possible care and service to you and regard your understanding of your financial responsibilities as an essential element of your treatment. Your insurance company may require that your primary care physician direct your care. If a referral form is needed, it is important that you contact their office prior to your appointment. It is your responsibility to ensure that our office has a current referral for your upcoming visit.
Insurance Claims
We will file your insurance claims for you. You will be responsible for any deductible, co-insurance, and services not covered by your insurance plan. In addition, if your insurance plan requires a co-payment, it will be due at the time of your visit. If additional tests are required as a part of your examination, you will be billed separately for any services provided by the hospital, anesthesiologist, laboratory and radiology.
Please remember that your insurance policy is a contract between you and your insurance company. While we will assist you by filing your claims, it is your responsibility to pay any balance due upon receipt of a statement from our office. We appreciate your feedback.
